Allegany/Western Steuben Rural Health Network , Allegany County, NY: $6,000 to expand the availability of Powerful Tools for Caregivers to benefit caregivers of Allegany County. The funds will cover the training of class leaders and replacement leaders over a two-year period.
Alzheimer's Association of WNY Williamsville, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve transitions from hospital to home for people with dementia using a transition coach.
Alzheimer's Association of WNY Williamsville, NY:  $100,000 to develop a practical DVD to assist nursing home staff and family caregivers in preventing and managing the behavioral problems of Alzheimer's patients and to pilot test it in several Buffalo nursing homes.
Alzheimer's Association WNY Chapter , WNY: $30,000 to serve as the team leader for a Quality Team participating in the Transitions of Care Collaborative. This team will work to improve transitions by working with caregivers to prepare them for future transitions of their family member.
American Red Cross Greater Buffalo Chapter $25,000 to support the Life Project that engages young people as volunteers through their schools to help seniors that require assistance to remain in their homes.
American Red Cross Greater Buffalo Chapter , Western New York: $12,000 to aid in the implementation of the Neighborhood Action Initiative by supporting the grantees in meeting their grant application objectives.
ARISE Syracuse, NY: $149,945 for counseling for children and families at two Syracuse School District elementary schools.
Baker Victory Services , Erie County, NY: $21,000 to use in collaboration with Catholic Charities to present a workshop and "Train the Trainer" session with national experts Dr. Robert Anda & Dr. Sandra Bloom on the "Impact of violence on Children's Health" throughout their lives. This grant is part of the Fellows Action Network.
Bassett Healthcare as fiscal agent for the NYS School Based Health Center Coalition: $19,900 to complete a report to inform schools in the 16 counties served by CHFWCNY on the school-based health care model including expert recommendations for dissemination of the information.
Bassett Healthcare (for NYS Coalition for School-Based Health Centers , Central New York: $86,435 to implement a strategy that will educate and inform CNY school districts with high percentages of children in poverty about the SBHC model. Interested districts are then eligible for additional resources to determine the viability of this model in their district.
Boys & Girls Clubs of Buffalo Buffalo, NY: $150,000 to provide dental services in partnership with the Buffalo Community Health Center to youth and families.
Buffalo Academy of Medicine- Buffalo, NY:  $7,500 for a facilitator to help solidify the planning and development needed to design a Regional Health Information Organization (RHIO) for Western New York.
Buffalo Ensemble Theatre Buffalo, NY: $88,000 to develop an interactive theater program for inner-city high-risk Buffalo teens focusing on health careers and health risks.
Buffalo Pediatric Society, Buffalo, NY: $7,500 to support the Buffalo Pediatric Society in their Vaccine Safety Awareness Radio Campaign.
C-Match Syracuse, NY: $149,995 to establish a Fetal and Infant Mortality/Morbidity Review/Registry (FIMMRR) for Onondaga County, providing critical feedback to healthcare providers, which can be used for Continuous Quality Improvement.
California Institute for Health Systems Performance , Western & Central NY: $8,000 to assist in developing a white paper intended to help CHFWCNY understand and consider options for expanding the use of a quality improvement collaborative (QIC) approach that will assist in achieving the goals for the Foundation's target populations.
Cattaraugus County Department of Aging Cattaraugus County, NY: $17,000 to support inclusion in the Sharing Your Wishes project to improve health-care decision-making for frail elders.
CCH Home Care and Palliative Services , Liverpool, NY: $11,308 to establish a home telehealth program. Through this grant, CCH will lease monitoring equipment for use with an estimated 50 Medicaid clientele who are long term patients with chronic conditions.
Center for Excellence in Health Care Journalism , Western & Central NY: $7,000 to provide scholarship support for WNY and CNY health reporters to participate in the Association's Urban Health Journalism Workshop, to be held in New York City on October 12 & 13, 2007.
Center for Health Improvement Sacramento, CA: $53,515 to identify current models of care for children living in communities of poverty.
Center for Hospice & Palliative Care Cheektowaga, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to raise the awareness of end-of-life options for adults with developmental disabilities and reduce unnecessary transfer to higher levels of care.
Center for Hospice & Palliative Care Cheektowaga, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve the transition to end-of-life care for appropriate patients in a medical practice.
Center for Hospice & Palliative Care Cheektowaga, NY: up to $4,000 for successful participation in the Community Health Foundation’s Quality Improvement Collaborative to Benefit Frail Elders.
Central New York Regional Market Authority , Syracuse, NY: $21,000 to support the second year of operation for the Syracuse Mobile Market's Farm Fresh Mobile Market that provides fresh produce using a mobile market van service to neighborhoods in the City of Syracuse where no supermarkets exist.
Chapin Hall at the University of Chicago , Central New York: $30,000 to develop an environmental scan leading to an actionable strategy for improving birth outcomes for children in poverty birth to one year of age in Central New York.
Chautauqua County Health Network , Jamestown, NY: $30,000 to support completion of an application for a Federally Qualified Health Care Center (FQHC) to provide primary, mental health and dental health care to the uninsured in Chautauqua County.
Chautauqua County Health Network , Jamestown, NY: $87,000 to provide scholarships for rural primary care providers who have significant numbers of frail elders in their practices to attend the Harvard Medical School Annual Review of Geriatric Medicine in 2009, 2010 & 2011.
Child & Family Services Buffalo, NY: $92,840 to implement Anasazi software to increase staff time to provide direct services to clients.
Ciurzak & Company Buffalo & Syracuse, NY: $115,000 to evaluate implementation of the Center for Disease Control's Coordinatored School Health Model in the Buffalo and Syracuse City School Districts.
Columbia University New York, NY: $150,000 to evaluate the Community Health Foundation’s Nuts & Bolts initiative and provide technical assistance to the initiative’s sixteen grantees.
Commission for a Healthy Central New York Syracuse, NY: $67,000 to support the eight county health departments in CNY efforts to organize and lead regional advocacy and development in four priority areas; obesity, mental health, dental health and stroke.
Community Care of WNY- $150,000 to support the early development for the first rural PACE site in New York State.
Community Care of WNY (Home Care & Hospice)- Olean, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to increase the ability of older patients with CHF and COPD to better manage their care and reduce avoidable hospitalizations and emergency care using a transition coach.
Community Catalyst, Inc. WNY: $10,000 to conduct key informant interviews and a visioning session to support the WNY Health Equity Workgroup's efforts to develop effective interventions that will reduce health disparities within WNY.
Community Concern of WNY Derby, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to improve the success for frail elders discharged from a TLC facility using a transition coach.
Community Dispute Resolution Center , Ithaca, NY: $5,600 to expand the Community Dispute Resolution Center's Wise Talk Program to include a peer referral component. Wise Talk offers mediation to elders, their families and the professionals with whom they interact.
Community Health Center of Buffalo, Inc. , Buffalo, NY: $10,000 to help facilitate the opening of a Federally Qualified Health Center satellite of the Community Health Center of Buffalo in Niagara Falls, NY.
Community Health Center of Buffalo, Inc. , Buffalo, NY: $10,000 to support the Community Health Center of Buffalo in better positioning itself to submit a successful proposal under the American Recovery & Reinvestment Act of 2009 Funding Opportunity: Facility Investment Program (FIP).
Concept Systems Ithaca, NY:  $17,650 to engage stakeholders from three neighborhoods in the City of Buffalo in a consensus building process that will identify priorities and guide the development of strategies to assist frail elders to age in place in the identified neighborhoods.
Continuing Care Foundation (an organization of the Catholic Health System), Buffalo, NY: $300,000 from the Community Health Foundation’s donor advised fund at the Community Foundation for Greater Buffalo for development of a Program of All-Inclusive Care of the Elderly (PACE) in Lackawanna, NY.
Cornell Cooperative Extension of Monroe County , Western & Central, NY: $174,829 to serve as administrative bookkeeping support for the CHFWCNY Health Leadership Fellows program. In December 2007, an additional $183,700 was awarded for continued support. An additional $42,000 was awarded to cover 2009 expenses.
Council on Aging of Niagara County for the Niagara Caregivers Network, Niagara Falls, NY: $69,519 to support a two year county-wide effort to improve health-care decision-making for frail elders. The project, known as Sharing Your Wishes has been extended for one year with an additional grant of $34,262. In May 2008 an additional $15,000 was awarded to fund measurement of this project through 2010.
Council on Aging of Niagara County Niagara Falls, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to initiative a self-management program for elders to reduce unnecessary hospitalization.
Crouse Hospital Syracuse, NY: Up to $30,000 to participate in the Community Health Foundation's Quality Improvement Collaborative to Improve Care Transitions for Frail Elders.  The team will work to reduce avoidable emergency room visits and hospitalizations as well as improve self-management using a transition coach.
Crouse Hospital Syracuse, NY: up to $4,000 for successful participation in the Community Health Foundation’s Quality Improvement Collaborative to Benefit Frail Elders.
Crouse Hospital Syracuse, NY: $41,550 for Crouse Hospital in collaboration with Aurora of CNY to develop a curriculum to be used with various levels of hospital staff to develop awareness and specific skills to use in accommodating patients with vision and/or hearing loss.
D'Youville College Buffalo, NY: $15,210 to plan and implement a conference designed to disseminate the results of the 2004 Health Risk Assessment to appropriate organizations, individuals and other interested parties.
Diocese of Buffalo Buffalo, NY: $73,043 to provide training to decrease bullying and increase cultural competency in the Diocese’s 20 most impoverished schools.
East Hill Family Medical Auburn, NY: $35,000 to expand dental services capacity by 20% through a combination of the grant from CHFWCNY and a $65,000 loan from the Primary Care Development Corporation.
Erie Niagara Area Health Education Center, Inc. , Buffalo, NY: $24,970 to support the planning process for "Bringing Health Literacy to Buffalo: A collaborative proposal."
