The Health Foundation for Western and Central New York is committed to improving the health and health care of the people and communities of western and central New York.

Our focus is on two of the most vulnerable and underserved populations in our regions: frail elders and young children living in communities of poverty. In support of both groups, we also invest in strengthening health capacity in our communities.

To achieve our vision, we collaborate with community partners in the 16 counties we serve to strengthen the health care system, promote education and advocacy and encourage positive individual behavior changes.

What's new at HFWCNY?
To celebrate the Health Foundation for Western and Central New York’s 10-year anniversary, we are pleased to share “Our First 10 Years 2002-2012.” In this report, we take a look back at where we have been as a Foundation, what we have accomplished together with the people and communities we serve and look forward to what is on the horizon.
